چکیده

autoimmune thrombocytopenic purpura, autoimmune haemolytic anaemia, systemic lupus erythematosus and rheumatoid arthritis. [2–4] The production of antibody depends on the antigenically driven clonal expansion of B cells into antibody-producing effector plasma cells. Plasma cells are short-lived cells, and survival of the populations depends on differentiation from their progenitor B cells [5]. B-cell ablation with rituximab could prevent further plasma cell and subsequent antibody formation, extinguishing the inflammatory anti-GBM response. In our case, the anti-GBM titre did not decrease below reference level even though 51 L of plasma volume had been exchanged, suggesting a high rate of antibody formation. The rapid decline of antibody titre and proteinuria could very well have been due to a late effect of simultaneous earlier therapy, but the theory behind the use of rituximab does not rule out that the rituximab might have had beneficial effect on antibody formation.

Clinical Effects of Topical Tacrolimus on Fox-Fordyce Disease

Fox-Fordyce Disease (FFD) is a rare, chronic, pruritic, inflammatory disorder of apocrine glands. It is characterized by dome-shaped, firm, discrete, skin-colored, and monomorphic perifollicular papules. The most common sites of involvement are axillae and anogenital and periareolar regions which are rich in apocrine sweat glands. Treatment is difficult. Topical, intralesional steroids, topical...

A Review on Therapeutic Drug Monitoring of Immunosuppressant Drugs

Immunosuppressants require therapeutic drug monitoring because of their narrow therapeutic index and significant inter-individual variability in blood concentrations. This variability can be because of factors like drug-nutrient interactions, drug-disease interactions, renal-insufficiency, inflammation and infection, gender, age, polymorphism and liver mass. Drug monitoring is widely practiced ...

Randomized trial of tacrolimus 0.1% ointment versus triamcinolone acetonide 0.1% paste in the treatment of oral pemphigus vulgaris

Background: Pemphigus vulgaris is a rare autoimmune disorder characterized by cutaneous and mucosal blistering. Surprisingly, the management of oral lesions has been detailed only infrequently. As current topical therapies for oral lesions are of limited efficacy, application of calcineurin inhibitors is considered to be a potential option. The aim of this essay was to investigate the efficacy ...

Detection of Clindamycin in Pharmaceutical Products using an Electrochemiluminescence Electrode based on a Composite of Ru(bpy)3²+, Eu2O3 Nanoparticle and Chitosan

In this study, the electrochemiluminescence (ECL) interaction of clindamycin and tris(2,2′-bipyridine)ruthenium(II) (Ru(bpy)32+) was used to develop a sensitive tool for the detection of clindamycin based on modifying a glassy carbon electrode (GCE) with nanoparticles of europium oxide. The experiments showed that in the presence of these inorganic nanoparticles (Eu2<...

Clinical Pharmacokinetics of Tacrolimus in Iranian Liver Transplant Recipients

Tacrolimus, a cornerstone of immunosuppressive therapy in solid organ transplantation, has a narrow therapeutic range with considerable inter-individual and intra-individual pharmacokinetic variability. To date, there is no information on the pharmacokinetics of tacrolimus in Iranian liver transplant recipients. This study was designed to determine pharmacokinetic properties of orally administe...
